Malayalam AI Voice Agent for Clinics and Hospitals in Kerala

A Malayalam AI voice agent for a Kerala clinic or hospital handles the logistics calls: it calls new enquiries within about 30 seconds, books a slot on Google Calendar, reminds patients before the visit and answers Ayurveda or medical-tourism enquiries from the information the clinic loads. It speaks Malayalam with English words mixed in, and it never gives medical advice. Booking, reminders and enquiry follow-up are what it does well; diagnosis, dosage and triage are not.

Where the line sits

The agent books, reminds and collects logistics. It does not diagnose, name medicines, read reports or judge urgency. Anything that sounds like an emergency should be told to go to the nearest hospital or call 112, and the call is logged for the clinic team. This guide is general information, not legal or medical advice.

What a Kerala clinic or hospital phone line actually carries

Most calls to a hospital front desk are not clinical. They are questions about OP timings, whether a doctor is in today, which token number is running, when a scan or lab report will be ready, and whether tomorrow morning is free. Those are the calls an AI voice agent can take, and they are the ones that bury a small reception team.

Kerala adds a few patterns of its own. The person calling is often not the patient: a son in the Gulf books for his mother, or a daughter rings on behalf of her father after a family discussion. Calls bunch in the evening and on Sundays when relatives are at home. And the spoken language is Malayalam with English nouns, such as OP, token, scan, report, ECG and appointment.

What a Malayalam clinic call sounds like

A reminder call opens in Malayalam, and the English words stay in English because that is how patients say them. An example for an imaginary clinic follows, with the English meaning beneath.

(Namaskaram, I am the AI assistant of Example Clinic. I am calling to remind you of tomorrow's 10:30 morning appointment; will you be able to come?)
  • —Caller: നാളെ പറ്റില്ല, വെള്ളിയാഴ്ച ആയാൽ കുഴപ്പമുണ്ടോ? (Tomorrow is not possible; would Friday be all right?)

Words and numbers to test before launch

Say times the way Kerala callers do: 'പത്തര' for half past ten, 'ഒന്നര' for half past one, and a mix such as 'eleven മണി'. Say the patient's relationship in the same breath as the booking: 'അമ്മയ്ക്ക് വേണ്ടിയാണ്' (it is for my mother). The agent must record the patient's name and not the caller's.

Department names are another test. Cardiology, ortho, ENT and gynaec arrive in English or in a clipped form, and place names such as Kozhikode and Calicut should resolve to the same branch. Read the transcript of each test call, because what the agent understood matters more than what it said.

Booking on Google Calendar: what it does and does not do

Booking runs through one tool. The agent confirms a date and time aloud, passes the patient's name, phone number and reason, and the system books the slot if it falls inside the business hours you set and is free. If the slot is taken, the agent says so and offers another time.

It is booking only. There is no reschedule or cancel tool, no list of free slots read out in advance and no invitation email to the patient. A change of plan is handled by staff or by a fresh booking, and the confirmation message to the patient comes from a separately configured WhatsApp template. Times are treated as Indian Standard Time.

WhatsApp reminders: approved templates only

Want to hear this working, not just read about it?

A WhatsApp message after a call is a pre-approved template sent through the clinic's own account on Meta Cloud API, Interakt or AiSensy. Outpero does not send from its own number. The template carries text and variables such as the patient's name and the appointment time. It does not carry images, report files, location pins or buttons, so the clinic address goes in as plain text.

Wording deserves care. Family members often share a phone, so a template should say 'your appointment at Example Clinic is tomorrow at 10:30' and leave the department, test name or diagnosis out. Template approval by Meta takes minutes to hours, so plan it before launch. See the page on WhatsApp messages during and after calls for the setup.

No medical advice: how to write the boundary into the agent

  • —State the limit in the script: the agent says it can help with appointments and information about the clinic, and that a doctor must answer medical questions.
  • —List red-flag words in the instructions, such as chest pain, breathlessness, heavy bleeding, fainting and severe injury. On any of these the agent tells the caller to go to the nearest hospital or call 112, and the call is logged for staff.
  • —Forbid the agent from naming medicines, doses, test results or reasons for a symptom, and from saying whether a report is normal.
  • —Load only facts the clinic has approved: timings, doctor names, fee ranges, locations and package contents. An answer that is not in the knowledge base becomes 'a staff member will call back', never a guess.
  • —Test five prompts before launch: a chest-pain call, a question about combining two tablets, a request to read a report aloud, a fever in a child and a question about the cost of an unlisted procedure.

Health information is among the most sensitive data a business holds, and India's Digital Personal Data Protection Act, 2023 applies to personal data generally. The clinic is responsible for how it collects, stores and uses patient data, and these choices keep that footprint small. This is general information, not legal advice.

  • —Capture logistics only: name, phone, department, preferred day and reason for the visit in the patient's own words. Do not set up capture fields for diagnosis, history or medicines.
  • —Verify who is on the line before saying anything specific. Confirm the name, and give nothing beyond the time and place of an appointment until the right person answers.
  • —Turn on the recording notice if calls are recorded, so the caller hears that the call is being recorded before the greeting.
  • —Caller memory is off by default and keeps the last five call summaries per number for that agent. Switch it on only if the clinic wants returning patients greeted by name, and use the forget-one-number control when asked.
  • —Webhooks can exclude the transcript, so the clinic management system receives the outcome and the captured fields without the full conversation.
  • —Keep a do-not-call list. Calling windows should stay inside sensible hours, and a patient who asks to stop gets added to the list.

Ayurveda and wellness centres: extra care on claims

Ayurveda centres in Kottayam, Thrissur, Kannur and Thiruvananthapuram attract enquiries from far outside Kerala, many of them started by a form or an ad. Callers ask how many days a Panchakarma programme runs, whether accommodation is included and what a course costs. Those are answers the centre can give in advance.

India restricts advertising of health claims, including under the Drugs and Magic Remedies (Objectionable Advertisements) Act, 1954. The agent should therefore describe what a programme includes, never what it cures. Check the wording of the knowledge base with your own legal adviser.

A worked month for a mid-size Kochi clinic

Illustrative arithmetic, not a quote. A clinic sends 800 reminder calls of about 1 minute and makes 300 enquiry callbacks of about 3 minutes. That is 800 plus 900, or 1,700 connected minutes. At ₹5 a minute on Standard voices that is ₹8,500.

Add ₹649 for the number and the total is ₹9,149. GST at 18% is about ₹1,647, for a monthly figure of roughly ₹10,796. Calls that never connect are not billed. A receptionist's salary at the ₹18,000 reference figure stays on the books for other work; the calling volume simply no longer lands on the desk.

Honest limits

  • —Voicemail detection is tuned for English greetings, so a Malayalam voicemail greeting may sometimes be missed. Read early call logs.
  • —Dialect and accent vary from Thiruvananthapuram to Kasaragod. Outpero uses a neutral spoken register and does not train per dialect, so test with patients from each of your districts.
  • —Elderly patients on poor lines, or with strong regional speech, are the hardest case. Offer them a human callback in the script.
  • —Callers abroad on foreign numbers cannot be dialled, and Tulu is not supported.
  • —Inbound answering on your own number is a mode that is launching, so confirm its status for your account before building a front-desk plan around it.
  • —The agent is a booking and information tool, not a clinical one. A clinic that wants more than that should keep a human on the line.

A two-week pilot

  1. 01Load clinic timings, doctor list, fee ranges and one page of FAQs as knowledge.
  2. 02Set business hours, visit length and the Google Calendar connection, then run the test booking.
  3. 03Write the no-advice boundary and the red-flag list, and test the five prompts.
  4. 04Get one reminder template approved on the clinic's WhatsApp account.
  5. 05Call 30 patients from one district who have agreed to be called, and read every transcript.
  6. 06Compare no-shows and unanswered calls with the previous fortnight, then widen the list.

Frequently asked questions

Can a Malayalam AI voice agent book hospital appointments in Kerala?

+

Yes. After confirming a date and time aloud, the agent books it in the clinic's Google Calendar with the patient's name, phone and reason, inside the business hours you set and only if the slot is free. It is booking only: there is no reschedule or cancel tool, so a change is handled by staff or a new booking.

Does the AI agent give medical advice?

+

No. The agent is instructed to handle appointments and clinic information only and to say a doctor must answer medical questions. On red-flag words it tells the caller to go to the nearest hospital or call 112, and the call is logged for staff. Test chest pain, tablet-combination and report questions before launch.

Does it understand Malayalam mixed with English?

+

Yes. It opens in Malayalam and follows the caller into English mid-sentence, which is how patients say OP, token, scan, report and appointment. Run the test with your own patients, from each district, because accents differ across Kerala and Outpero does not train per dialect.

Can it send WhatsApp reminders to patients?

+

Yes, as pre-approved template text sent through the clinic's own Meta Cloud API, Interakt or AiSensy account. Messages carry text and variables such as the name and time, not images, report files, location pins or buttons. Keep the wording free of clinical detail, since family members often share a phone.

Can it handle Ayurveda and medical-tourism enquiries?

+

Yes, for logistics. It explains package contents, duration and what to bring from text the centre loads, and captures country, treatment area and best time to call. It makes no claim about outcomes. It dials Indian numbers only, so an enquirer abroad has to be reached through a relative's Kerala number or by a person.

How much does it cost for a clinic?

+

Calls are ₹5 a connected minute on Standard voices or ₹6.8 on Premium, with no setup fee and no subscription. The number is ₹649 a month with one channel. As an illustration, 800 one-minute reminders and 300 three-minute enquiry calls come to about ₹10,796 with 18% GST. New accounts get 50 free credits.
